Knowledge, Attitude, and Practice of Fluoride Use among General Dental Practitioners in Government Hospitals, Khartoum State

Background: Fluoride therapy delivered topically or systemically, is the most effective method for preventing dental caries. Aim: This study aimed to assess the knowledge, attitude, and practice (KAP) of general dental practitioners (GDPs) regarding fluoride application in government hospitals in Khartoum State. Materials and methods: An analytical cross-sectional facility-based study was conducted among GDPs across three government hospitals. A sample size of 185 participants was targeted, with 173 valid questionnaires included for analysis. A structured, pretested questionnaire was used to collect data on socio-demographics, KAP related to fluoride application. Data were analyzed using SPSS version 28. Informed consent was obtained from all participants. Results: Overall knowledge of fluoride applications was moderate. Nearly half (49.4%) of participants understood the long-term effects of fluoride. However, practitioners demonstrated a negative attitude; only 27.7 and 23.7% were familiar with the role of fluoride in toothpaste and water systems in caries reduction, respectively. Clinically, 58.8% performed fluoride treatments, with only 35.3% conducting ten or more applications per month. A significant correlation was found between age and fluoride knowledge, with older practitioners showing greater awareness. Patient-related factors were the most common barrier to application. Conclusion: The study revealed moderate knowledge, negative attitudes, and poor clinical practices regarding fluoride use among GDPs. Recommendations: Continuous in-service training, updates in dental school curricula, standardized demonstration and documentation of fluoride application procedures, and the establishment of a supervisory system are recommended to improve clinical practice of fluorides.
