Abstract

Introduction: Palatal fistulas can be managed conservatively or surgically depending on the size of defect. Giant fistula is usually treated with local flaps or local flaps combined with regional flaps. Tongue flaps are an excellent alternative to close wide or recurrent palatal fistulae. Here we report a case of a giant palatal fistula postcleft palate surgery which had been operated thrice and failed in different centers. This fistula has been successfully repaired in our center with posteriorly based tongue flap. Keywords: Palatal fistula, Cleft palate, Tongue flap.

Abstract

Down syndrome or trisomy 21 is a chromosomal abnormality. Presence of neonatal or pre-deciduous teeth is commonly seen in downs syndrome. According to published literature maxillary and mandibular anterior teeth are commonly seen. Presence of Rege-Fida disease in infants with Down syndrome due to mandibular anterior neonatal teeth and secondary ulceration is well known, however presence of posterior neonatal teeth is rare. Here by we present a case report of a 48 day old child with Down’s syndrome with pre-deciduous dentition in the maxillary posterior region. Keywords: Downs Syndrome, posterior Natal tooth, Neonatal tooth

Abstract

Sialolithiasis is the obstruction of salivary gland due to blockage of its excretory duct in response to the formation of calcareous concretions which causes the dilation of salivary gland. Sialadenitis is an acute infection of salivary gland. Sialadenitis is seen mostly in parotid gland and sialolith is mostly reported in sub-mandibular gland. We hereby report a case of elderly female with an acute pain on left sub-mandibular region which was diagnosed as sub-mandibular sialolithiasis leading to sialadenitis. Keywords: Sialolithiasis, Sialadenitis, Calculi, Conservative management.

Abstract

Craniofacial Trauma in pediatrics and adults shows almost similar pattern of injuries. Incidence and prevalence of pediatric craniofacial trauma is comparatively less than adults. In pediatric trauma craniofacial injuries are much observed due to the large cranium is to body ratio. Management of pediatric trauma case becomes more difficult due to obvious reasons as there is less incidence of pediatric traumas which indeed lacks our experience to manage it. Pediatric craniofacial trauma requires a multidisciplinary team approach for the better management. In this case report we present a 3-year-old male child with a craniofacial trauma due to natural disaster of wall collapse. Keywords: Pediatric, Craniofacial, Trauma, Management, Multidisciplinary.

Abstract

Lingual node metastasis is seen most commonly in carcinoma of tongue and carcinoma floor of mouth. Literature elicits about such cases, but rarely reported is lingual node metastasis in Squamous cell carcinoma of alveolus. Our case report is an elderly male patient with Squamous cell carcinoma of alveolus. Management included hemi-mandibulectomy because of the extension. Metastatic lingual node was found during the surgery as it is rare finding so is this case report presented to reappraise the literature. Abstract

Mandibular first premolar is often considered as enigma to endodontist. Though most frequent type of tooth configuration reported is the presence of one root and root canal.  This case report enlightens the prime aspects of diagnosis and endodontic management of an aberrant configuration with respect to mandibular first premolar. A variation in number of roots as well as canal morphology is not scarce. According to various studies on anatomic configuration 1.8% of mandibular first premolar showed two roots. Root canal morphology can be unpredictable and requires cautious assessment for root canal therapy. This article reports a clinical case of mandibular first premolar with two roots and root canals treated successfully with endodontic therapy. Keywords:Mandibular first premolar, Aberrant morphology, Endodontic management

Abstract

The most widely recognized difficulty of a repaired congenital fissure is palatal fistula which can be seen from narrow to wide forms as derived in the published literature. Small fistulas are usually asymptomatic, but the larger ones create regurgitation of fluids in the nasal cavity and interferes the normal speech. Various options are present for the surgical management to close fistulas. Local flaps are usually used for small fistulas comparatively large fistulas are difficult to manage. Tongue flaps due to its high vascularity are region is the treatment of choice for most of the surgeons. The anteriorly based tongue fold is a sheltered and compelling strategy for conclusion of moderately extensive repetitive palatal fistula with no practical impedance of the giver site. This article portrays remaking of one such wide palatal fistula by anteriorly based tongue fold performing single layer conclusion. Keywords: Palatal fistula, Tongue flap, Cleft palate, Local flap

Abstract

Aim: To evaluate the changes in blood pressure, pulse rate and SpO2 pre during and post extraction procedure under local anesthesia. Materials and Method:A total of 150 patients were included in the study. Pulse oximeter was placed to evaluate the parameters and changes were investigated before administering local anesthesia, during anesthesia, during extraction procedure and post extraction. Data was tabulated and statistical analysis was done using 20 SPSS software. Results:Almost of the patients has increase in the systolic blood pressure and pulse rate during administration of local anesthesia and during extraction procedure. Very minimal change was observed in diastolic blood pressure and SpO2 levels. Conclusion:Significant changes are noted in changes in systolic blood pressure and pulse rate pre administration of local anesthesia, during administration of local anesthesia, during extraction, and post extraction procedure. Diastolic blood pressure and SpO2 has no significant differences. Keywords:Blood pressure, Pulse rate, SpO2, Dental extraction, Local anesthesia
