Abstract

Objectives: The aim of this study was to determine most common cause of orofacial pain and gender distribution. Materials and Methods: This was a prospective study that included 200 patients that reported to our Department of Oral Medicine and Radiology with orofacial pain. Results: Out of 200 patients 130 (65%) were females and 70(35%) were males with age range of above 15 to 65 year. Temporomandibular disorders were most common cause orofacial pain .58 cases (29%) out of 200 were temporomandibular disorders with 38(19%) cases of females and 20(10%) cases of males followed by myofacial pain in 48% cases and least incidence of traumatic neuritis in 3% cases. Conclusion: Chronic orofacial pain is a diagnostic challenge and it is possible therefore to make a misdiagnosis Multidisciplinary OFP assessment ideally also includes psychometrics, pain profiling, quantitative sensory testing, haematology and imaging where indicated. Keywords: Orofacial pain, Neuropathic pain, Nociceptive pain, Somatosensory.

Abstract

Introduction: Temporomandibular disorders (TMD) is a term used to describe collectively a number of related disorders affecting the temporomandibular joints, masticatory muscles, and associated structures. The treatment modalities include surgical and non-surgical methods. In this study non-surgical method was used. The aim of present study was to evaluate and compare the efficacy of topical and systemic NSAIDs in the management of pain of TMD origin. Materials and Methods: A single blinded cross-sectional study was conducted and a total of 40 clinically diagnosed cases of TMDs of muscular origin were selected. Once the diagnosis of myalgia due to TMD was made, the patients were randomly allocated in one of the treatment groups. The study divided the participants into 2 categories: GROUP A: oral diclofenac 50mg, and Thiocholchicocide 4mg 12 hourly for 15 days. GROUP B: 200mg of transdermal patch of diclofenac to be applied over the masseter muscle 12 hourly and Thiocolchicoside 4mg twice daily as in group A for the period of 15 days. Results: Females where most commonly affected with a ratio of 2.07:1 and a mean age of 33.5 years. For group A mean verbal rating score of pain was 8, 5, 4, 1 on 3rd, 7th, 15th, and 21ist day respectively and group B mean verbal rating score of pain was 7, 5, 3, 1 on 3rd, 7th, 15th, and 21ist day respectively. There was no significant difference in trends of pain intensity in patients treated by oral NSAIDS and patients who were treated with Transdermal patch group. Conclusion: There was no significant difference in trends of pain intensity in patients treated by oral NSAIDS and patients who were treated with Transdermal patch group. However treatment with topical NSAIDs has been shown to provide clinically effective analgesia at the site of application while minimizing systemic absorption. Keywords: Transdermal diclofenac patch, Thiocholchicoside, Gamma amino butyric acid, Temporomandibular joint disorders.

Abstract

Introduction: Present study was undertaken to evaluate application of swish and spit [elixir] technique of drug delivery for management of patients with multiple aphthous ulcers especially in large posterior lesions where topical appilication is difficult and cumbersome. Aim: The aim of present study was to check efficacy of tetracycline and prednisolone elixir in treatment of recurrent aphthous ulcers and check which drug among the two is better for prevention of recurrence of the lesions. Materials and Methods: A total of fifty patients were selected with  recurrent aphthous stomatitis and divided in two groups Group A and Group B. Group A patients were asked to use tetracycline elixir and Group B patients used prednisolone  elixir thrice a day for 15 days.  Pain Severity, ulcer count and size of the ulcers were determined. Results: There was no statistically significant difference in study parameters Viz; pain Severity, ulcer count and size of the lesions in both group. However rate of recurrence of lesions was much low in prednisolone group as compare to tetracycline group. Conclusion: At the end of study it was concluded that both tetracycline and prednisolone elixir can be used for treatment of recurrent aphthous stomatitis.  However prednisolone was better in terms of recurrence of lesion with less percentage of recurrent lesions. Keywords: Tetracyline, prednisolone, swish and spit, elixir, multiple recurrent aphthous ulcers
