Abstract

Objective: The aim of this study is High risk pregnancies & implications of color Doppler. Total 100 were selected with purposive sampling technique, (50 cases) study group of high risk pregnancy taken into the study & compared with an equal number of women with normal pregnancy without complication. Result: In our study we found, maximum cases in age group of 24-34 i.e. 49% in group A whereas in group B 43% in more than 34 age group. 41% normal in group A whereas 27% in group B. C-section 79% in group a & 21% in group B. Emergency caesarean section was uncommon in subjects with normal Doppler USG than those of with abnormal Doppler USG and also from group B. Conclusion: In high risk women like pre eclampsia, diabetes & IUGR. Color Doppler flow velocimetry done repeatedly can predict address foetal events with a great degree of accuracy. Results obtained in study where clinically & statistically significant. Every tertiary hospital should routinely make use of facility for the assessment of fetal wellbeing in high risk cases & ensure a better perinatal outcome. Color Doppler can be used as most effective for fetal surveillance in high risk pregnancy cases. Most importantly it helps in guiding early intervention and outcome.   Keywords: High risk pregnancy, Color Doppler, Fetal outcome, pre-eclampsia, Intra uterine growth retardation, Perinatal outcome

Abstract

Background: During pregnancy one of the most common endocrinological disorder which are very often overlooked during pregnancy because of their nonspecific symptoms and the hypermetabloic state of pregnancy is Thyroid disorder. Optimal maternal thyroid function during pregnancy is important for both the mother and fetus. Objectives: The study was planned to determine the prevalence of hypothyroidism using the classical reference range of normal serum TSH as 0.4-6.2?IU/L. It also aimed to study the foetomaternal outcome among pregnant hypothyroid women, using the classical reference range and a lower cut off ( Materials and Methods: This was a longitudinal prospective study conducted in the Department of Obstetrics & Gynaecology, Suryanagari Hospital, in collaboration with the Departments of Endocrinology, Laboratory Medicine & Paediatrics. Before starting the study, clearance was taken from ethical committee of the hospital. A total of 522 pregnant women attending the antenatal OPD were recruited to determine the prevalence of hypothyroidism on the basis of TSH evaluation. Conclusions: With our study we concluded that these very high prevalence of hypothyroidism in India and routine maternal thyroid function testing must be done as soon as pregnancy is confirmed thus saving lives of many infants who could lead rest of their lives happily and prosperously.   Keywords: Foetomaternal, Hypothyroid women.

Abstract

Introduction: Pain management is an essential component of care provided by pediatric anaesthesiologists. That specially holds true due to inability of this group of patients to report their overall experience of surgical procedures like adults. Caudal analgesia is one of the most popular regional anaesthetic techniques employed in children undergoing surgery for lower abdominal, urological, and lower limb operations. Of all the major advantages that this technique has, there is one major disadvantage of short duration of anesthesia. In children it is observed that combination of ropivacaine and clonidine administered caudally has shown to prolong the effects of analgesia. The present clinical study is therefore undertaken to compare caudal ropivacaine with clonidine and ropivacaine alone with regards to hemodynamic changes, analgesic potency and side effects in children. Materials and Methods: This study included 60 children of the age group 5-10 years ASA grade I and II, of either sex, coming for various elective infra-umbilical surgical procedures who were divided into two groups each comprising of 30 subjects. Group A received plain 0.20% Ropivacaine (1ml/kg) and Group B received 0.20%Ropivacaine (1ml/kg) with clonidine 1µg/kg and the effects were analysed on various parameters. Results & Conclusion: In our study, we chose 0.20% ropivacaine which provides better quality of analgesia and clonidine 1.0?g/kg which prolongs the duration of analgesia significantly while avoiding the side effects like excessive sedation and bradycardia associated with higher doses. Other hemodynamic parameters did not differ significantly in both the groups.   Keywords : Pediatric anesthesia, Caudal block, Ropivacaine, Clonidine, Anesthetic complications, Local anesthetic agents.
