Abstract

Aim: To study the effectiveness of Nd: YAG laser peripheral iridotomy (LPI) in whole spectrum of primary angle closure disease including primary angle closure suspect (PACS), primary angle closure (PAC) and primary angle closure glaucoma (PACG). Materials and Methods: It’s a retrospective analysis of 171 eyes (87 patients) of primary angle closure disease who have undergone laser peripheral iridotomy (LPI) after proper prelaser evaluation. Patients with prior LPI, glaucoma surgery and secondary glaucoma were excluded. Results were analysed for IOP control, progression of disease and need of antiglaucoma medicine or surgery following LPI. Results: Out of 171 eyes of angle closure diseases, PACS was observed in 23 eyes, PAC in 51 eyes and PACG in 97 eyes. Overall progression was 23.97% (41 amongst 171 eyes) and none of the eyes with PACS shown progression. In PAC group, 5 eyes (9.8%) progressed to PACG and were managed on one antiglaucoma drug. In PACG group of 97 eyes, 36 eyes (37.11%) progressed and managed surgically while 61 (62.89%) eyes did not show any progression. Out of 61 non progressed PACG eyes, IOP was controlled without medication in 14 eyes (29.2%), with one anti glaucoma drug in 17 eyes (27.8%), and with 2 or more drugs in 30 eyes (49.18%). Conclusion: LPI is a very good treating modality, especially in PAC & PACS cases, but in PACG with medical treatment, it is a good tool. Majority of the patients included in the study did not show any disease progression and remained stable following LPI. Keywords: Nd: YAG laser iridotomy, Primary angleclosure glaucoma, Primary angle closure suspect & Primary angle closure.

Abstract

Aim: To compare the results of anterior flap suturing with excision of posterior flaps and both anterior & posterior flaps suturing in external DCR Materials and Methods:Total 46 patients were divided into 2 groups, 23 in each. In Group A cases with anterior flap anastamosis were included and in group B cases with double flap suturing were included. Comparison was done in terms of ease of procedure, total surgical time, recurrence and success rate. Mean surgical time was evaluated in both the groups. Results: Mean age in years was 53.56 7.19. Suturing the flaps was found convenient and easy in Group A (91.30%) in comparison to group B (13.04%). Total mean surgical time was 55.09 3.96 minutes. In Group A it was 51.43 0.99 minutes and in group B it was 58.74 1.78 minutes. In Group A mean surgical time was less in comparison to group B and it was statistically highly significant. Success rate assessed by patent syringing, was 91.30% in Group A and 95.65% in Group B. Conclusion: Only anterior flap suturing with trimming of posterior flaps in comparison to double flap anastamosis makes external DCR an easy and less time consuming procedure. Abstract

Aim: To find out the correlation of working hours on computers and severity of dry eye disease in bank employees. Materials and Methods: Study was done on in total 300 bank employees in the age group of 25 to 45 years. All the cases were divided into 3 groups depending on duration of working hours on monitor, Group A (0 hours to ? 3 hours), Group B (>3 hours to ? 6 hours) and Group C (> 6 hours to 9 hours). No statistically significant difference was found gender and age wise amongst 3 groups. In Group A, mean age was 29.35 ±5.5, in Group B it was 30.17 ±5.51 and in Group C it was 30.28 ±5.52). On the basis of OSDI Questionnaire, Schirmer’s test and tear film break up time (TFBUT), severity of dry eye was assessed. Results: With OSDI Questionnaire mild to moderate dry eye was found in (GR A 12% < GR> Conclusion: OSDI questionnaire combined with Schirmer’s test and TFBUT is a useful tool to assess dry eye disease. OSDI is a very important tool for functional assessment. Simple remedies like position of monitors and frequent blinking can improve the work efficiency. Keywords: Dry eye disease, Schirmer’s test, Tear film break up time (TFBUT), Computer vision syndrome & visual display terminus.
