Abstract

A holistic and multidisciplinary approach is required for ovarian cancer diagnosis and management. Ovarian tumor diagnosis is based on clinical symptomatology followed by radiological confirmation. Tumor markers are considered just adjunct. Management of ovarian mass is surgical and intra -op frozen section aids in understanding the nature of the mass but histopathology is definitive. Ovarian torsion affects 2.5 to 7.4% of all ovarian tumors. The present case will highlight the destructive changes brought about by torsion which could ultimately result in missed tissue diagnosis.   Keywords: Histology, Ovarian cancer, Torsion, Necrosis

Abstract

In the growing era where safe abortion practices are advocated, we still encounter the complications of illegal abortions. We present a case of 40-year-old P3L3A1 referred to our tertiary care center with history of massive bleeding following illegal second trimester abortion and USG revealing the retained fetal skull. The retained fetal skull was removed but there was torrential hemorrhage on attempting to remove the placental mass. Balloon tamponade and hemostatic agents failed leading to laparotomy and discovery of placenta accrete syndrome which eventually lead to hysterectomy, Intensive ICU care and admission, massive blood transfusion and ultimately contributing to maternal near miss situation. Keywords: Second trimester abortion, Massive hemorrhage, Retained skull, Placenta accrete syndrome.

Abstract

Placenta accreta is a life threatening obstetric condition that requires multidisciplinary approach for its management. Presently we document one such case of placenta accreta diagnosed intraoperatively during caesarean section where patient continued to bleed despite peripartum hysterectomy and eventually hemostasis was achieved by focused vault compression. Keywords: MRI, Posterior placenta accreta, Ultrasonography, Peripartum hysterectomy, Umbrella packing, Abdominal packing
