Abstract

Background: Tibial condyle fractures become more challenging when they present with damaged soft tissue. Our aim is to evaluate such fractures with Ilizarov ring fixator along with our modified technique to give better outcome. Materials and Methods: Our study included 54 cases with proximal tibia fracture treated with Ilizarov by same surgeon and team from December 2017 to May 2021. Results: Mean time for radiological union was 24.37 weeks (15 to 35 weeks). Ilizarov fixator we have removed 21 days after radiological union. Functional score was calculated using two scores: Hohl and Luck score, and Lyshom’s score. 12 has excellent, 28 good, 14 were fair and no patient with poor result. Mean Lyshom’s score was 81.73. Knee range of movement in maximum patients were 0 to 110 degree. There was few complications, like, Extensor lag which got corrected with physiotherapy. Pin tract infection which healed with few dressings. Conclusion: Ilizarov ring fixator is an excellent option for Tibial Plateau fractures as a definitive surgery especially with damaged soft tissue. And with our modification we can achieve perfect joint alignment and early weight bearing.   Keywords: Ilizarov fixator, Tibial condyle.

Abstract

Aim: The purpose of the study was to evaluate the functional and radiological results of bone grafting combined with cementing with a gel foam mettle (sandwich technique) in osteolytic lesions around the knee, which include Giant cell tumors and Chondromyxoid fibroma, and Chondroblastoma. Materials and Methods: We had 14 patients of such tumours who were evaluated and treated with extended curettage phenol painting followed by reconstruction of the cavity using a layer of gel foam followed by bone graft and bone cement. The modified technique which we followed included gel foam followed by bone graft followed by another layer of gel foam then bone cement. The period of study was from 2015 to 2019. With average follow up of 2 years. Result: The results were evaluated using the musculoskeletal tumor society scoring (MSTS) and average score in our patient was 24. There were no recurrence noted as lysis of more than 5mm at bone cement interface. Conclusion: We conclude that Modified Sandwich technique provided better prognosis and early resuming of activity. This technique with low rate of recurrence can be applied to all juxta articular tumors with good to excellent results. The only drawback of series was a short follow up. Keywords: Chondromyxoid fibroma, Chondroblastoma, Giant cell tumors, Modified Sandwich technique.
