Neurocysticercosis of the central nervous system is common of which extraparenchymal involvement is
less common. Racemose type is a rare and aggressive variety associated with increased morbidity. It is
characterised by presence of multiple confluent of cysts in the ventricles and subarachnoid space.
This is a case report of a 36 year old male, farmer by occupation who was diagnosed with racemose type
neurocysticercosis of the 4th ventricle with obstructive hydrocephalus. Emergency ventriculo peritoneal
shunting followed by excision of the cyst was carried out successfully.
Unlike parenchymal type, antihelminthes is contraindicated in intraventricular NCC prior to removal of the
cyst as it leads to ventriculitis. This case is being reported due to its rarity and paucity of literatures on
racemose type of neurocysticercosis affecting the 4th ventricle.
