A 47-year-old man presented with intussusception of the colon and was found to have a large tubulo-villous adenoma with high grade dysplasia, transforming into adenocarcinoma. The bowel adjacent to the polyp showed florid vascualr proliferation, mimicking a vascular neoplasm. However, there were no signs of atypia or malignancy in the vascular lesion. We illustrate an example of chronic obstruction inducing hypoxia, leading to neo-angiogenesis, histologically masquerading as a vascular neoplasm.
