Histocompatibility testing is pivotal to the transplantation process since it helps determine immune compatibility between donors and recipients. This compatibility reduces the risk of rejection and improves transplant outcomes. Errors in this testing can have serious consequences for patients and affect the survival of the graft. Therefore, maintaining a robust quality control (QC) system is essential. However, keeping robust QC standards in histocompatibility testing faces several challenges. These challenges arise from the diverse nature of human leukocyte antigens genes, advances in technology, and the lack of consistent interpretation standards among different laboratories. This review aims to compile current practices in QC in histocompatibility testing, including internal QC, external quality assurance, accreditation systems, and common challenges.
